Outpatient Records / Release of Information

We provide outpatient medical records to clinics, and upon request for patient appointments. Upon receipt of permanent change of station orders, medical records are packaged and mailed to the member's gaining unit. If a member is retiring or separating, medical records are mailed to Service Treatment Records Retirement. This process allows for future Veteran Affairs claims after retirement or separation. Routine record requests are usually completed in approximately 30 days. Retiring and separating active duty members need to request copies 180 to 45 days prior to separation to ensure adequate time to accomplish copies. Patients must provide an identification card and sign a Department of Defense Form 2870, Authorization of Release of Medical Record, in order for records or results to be copied or faxed, per Privacy Act and Health Insurance Portability and Accountability Act guidance.
