Dental
At the 45th Medical Group Dental Clinic, we are honored to support the oral health and readiness of our Active Duty service members. We provide general dentistry and limited specialty care services.
Who We Serve
This is an Active Duty-only dental facility. Eligible personnel include:
- Active Duty service members (all branches)
- National Guard and Reserve members on Active Duty orders for 30 days or more
Reserve members on Active Duty orders may receive exams, routine treatment and emergency care at our clinic. We are only able to treat Reserve members on active orders. Therefore, we will only begin treatment that can be completed during the activation period.
Family members, retirees, and other beneficiaries should seek dental care through the TRICARE Dental Program or the Federal Employee Dental and Vision Insurance Program. To explore available plans, visit TRICARE Dental.
Appointments & Scheduling
To make an appointment or ask dental-related questions, please call the front desk.
Appointment Guidelines:
- Arrive 10 minutes before your scheduled appointment
- If you are late, you may be considered a no-show
- Cancellations require at least two hours notice to minimize lost treatment time.
Dental Emergencies
Active Duty service members having dental concerns or emergencies during duty hours should call the clinic for an appointment.
For off-hours dental emergencies, please contact the After-Hours Emergency Line.
Referrals for Off-Base Care
- Referrals for off-base care are made through the clinic on a case-by-case basis for required treatment.
- If you receive off-base care, please ensure that our dental clinic receives the completed treatment notes to update your records.
Orthodontics (Braces)
- Our facility does not offer comprehensive orthodontic treatment.
- Members may elect to seek orthodontic treatment from a civilian provider at their own expense. The Air Force will not be responsible.
- Please contact us prior to initiating treatment to complete required paperwork.
Information for Members Out-Processing for Permanent Change of Station (PCS)

Information for Members Separating/Retiring
- You can request a copy of your dental records by providing a completed DD Form 2870 and a copy of your orders to the dental clinic.
- You can complete your dental out-processing by bringing the VA Memo from your out-processing packet and a copy of your orders to the dental clinic.

Non-Active Duty Access to Dental Care in the Community
TRICARE Dental Program
The TDP is a voluntary dental plan. Sponsors can enroll through the Beneficiary Web Enrollment website.
You can enroll if you're a:
- Family member of an Active Duty service member
- Family member of a National Guard/Reserve member
- National Guard/Reserve Member who isn’t on Active Duty or covered by the Transitional Assistance Management Program
United Concordia administers the TDP benefit. You'll need a Department of Defense Self-Service Logon to create your account. After you enroll, you can create an account with United Concordia to:
- View dental coverage
- Check a claim
- View claims history
- View explanation of benefits
- Register a chronic condition, pregnancy or special need
- Select communication preferences (e.g., text, email or mail)
- Access messages in a secure Messaging Center
