Physical Therapy

The 45th Medical Group Physical Therapy Clinic provides services to active duty members.  Patients must have a referral from a provider.  Please note that we have the right to retain rehabilitation services for active duty service members.  This means that you may be required to receive treatment in our clinic even if the same service is available at an outside provider.

Notes for your scheduled appointment:

• Please wear gym attire to your appointment to improve movement and access to the treatment areas.
• Please arrive fifteen minutes prior to your appointment time.
• Arriving early allows patients to accomplish paperwork and change clothes.
